Please use this page as a central resource for all Honors College forms, including those for graduation, senior thesis, internships, and undergraduate research.

The forms below can be viewed using Adobe Acrobat Reader™.  Most of these forms must be completed then delivered to the Honors College office.

Academic Forgiveness Application

Students wishing to apply for academic forgiveness must complete this form before enrolling in the forgiving class.

D.E.A.L. Application


Graduation Application

 All candidates for graduation must complete this form to ensure that the SC Honors College office knows which honors students will graduate each semester.

H-Option Form

 To H-Option a class that is not offered as an honors course, a student will complete this form in consultation with the professor, obtain the appropriate signatures, and submit it to the Student Services office (Harper 220).

Honors Residence Reservation Form

 Complete this online form if you or your organization would like to reserve the Honors Learning Center in the Honors Residence. Rooms are available for reservation Monday - Friday 5:00pm - 12:00 am

Independent Study Contract for Students

 Students participating in independent study programs sponsored by the South Carolina Honors College should complete this form. The form must be signed off by both supervisor and professor of record unless otherwise noted.

Internship Contract for Students

 Students participating in internship programs sponsored by the South Carolina Honors College should complete this form. The form must be signed off by both supervisor and professor of record unless otherwise noted.

Research Contract for Students

Students participating in Research programs sponsored by the South Carolina Honors College should complete this form.

SC Semester Program Application

Students applying to participate in the SC Semester internship programs will use this form.

SCHC 390z Pre-Enrollment Form

Complete and submit this form to enroll in SCHC 390Z (Senior Thesis Project Planning Course).

SCHC 499 Senior Thesis Contract

This form must be completed and turned in to the Honors College in order to enroll in SCHC 499 (or department equivalent).

Senior Thesis Binding Form


Senior Thesis Defense Confirmation

Two weeks prior to a senior thesis/project defense, this form must be completed and submitted with pre-defense copies of the thesis/project paper.

Senior Thesis Grant Application

Use this application to apply for grant money with the Senior Thesis Grant Form. Maximum grant given = $1,500.00.

Student Research Fellowship Application

The Honors College has a limited number of fellowships to facilitate students engaging in active scholarship under the mentorship of a faculty member. Open to all fields and disciplines. See Student Research page for detail on SURF and ESP program requirements.

Student Research Timecard

Science Undergraduate Research Fellowship and Exploration Scholars Programs Hours are to be reported on this time card and signed by the mentor. 

Hours not reported in a timely fashion(within 30 days) will not be paid. Hours reported may not exceed 150 per Fall or Spring semester (no more than 15 per week), or 300 per summer (no more than 40 per week). Timecards must be signed by your mentor in order to be processed.

 Students interested in participating in the Washington Semester Program should complete this application.
